Through this partnership, At Your Home Familycare will deliver in-home respite services for eligible individuals living with dementia, giving family caregivers essential relief while ensuring loved ones receive compassionate, professional care in the comfort of home.
A Growing Crisis Impacting Millions of Families
Dementia is one of the fastest-growing public health challenges in the United States. As the population ages, millions of families are finding themselves unexpectedly in the role of full-time caregivers — often while juggling jobs, children, finances, and their own health.
The emotional toll, physical exhaustion, and financial strain of dementia care are profound. Many caregivers experience burnout, isolation, and declining health, all while trying to keep loved ones safe and supported at home.
The GUIDE Model was created by the Centers for Medicare & Medicaid Services (CMS) to directly address this growing crisis by improving care coordination, expanding caregiver support, and providing access to respite services — at no cost for eligible Traditional Medicare beneficiaries.
For eligible Traditional Medicare beneficiaries, the GUIDE program provides up to approximately $2,500 annually in covered respite services at no cost, giving families meaningful financial relief alongside professional in-home care support.

Tembo Health serves as the clinical GUIDE provider, offering dementia evaluations, medical oversight, education, and ongoing care coordination. At Your Home Familycare complements this care by providing hands-on in-home respite services and daily living assistance.
