Vista Community Clinic Marks Diabetes Awareness Month with Access to Life-Changing Diabetes Care and Education

VISTA, CA – In honor of Diabetes Awareness Month, Vista Community Clinic (VCC) is emphasizing the importance of early intervention and comprehensive care for diabetes management. VCC’s Diabetes Self-Management Education and Support (DSMES) program offers essential services to support individuals in managing and preventing complications associated with type 1, type 2 diabetes, and pre-diabetes. Since launching in January 2024, VCC’s program has helped participants reduce their A1C levels by an average of 1.5%, significantly outperforming the national DSMES average of 0.6%.
At VCC, our clinicians and programs empower patients to gain control over their diabetes through expert-led education, personalized care plans, and comprehensive support. Patients gain tools and knowledge for managing their health, from understanding the symptoms and progression of diabetes to making sustainable lifestyle adjustments, with on-site care options.
“Diabetes Awareness Month” is a reminder that early intervention can make a significant difference in managing this chronic condition. With November marking Diabetes Awareness Month, VCC encourages community members to schedule a consultation to assess risk factors, understand the symptoms of diabetes, and explore effective strategies for self-management.
