VISTA, CA – Vista Community Clinic (VCC) is excited to announce that its Durian Senior Adult Care location has been recognized for delivering high-quality care tailored to older adults. This honor shows VCC’s dedication to making sure seniors get the safe, respectful, and individualized care they deserve, in an environment created specifically for them.

The clinic is part of the national Age-Friendly Health Systems movement, which works to improve healthcare for older adults by focusing on what matters most to them. VCC has adopted the 4Ms framework, a set of simple but powerful steps to ensure care is safe and effective:

• What Matters: Focus on each older adult’s goals and preferences, including their health priorities and end-of-life wishes.
• Medication: Use medications that are safe for older adults and avoid those that might harm their mobility or thinking.
• Mentation: Prevent, spot, and treat issues like memory loss, depression, or confusion.
• Mobility: Help older adults stay active and move safely every day to support their independence and health.
