VCC Launches Comprehensive Diabetes Self-Management Education and Support

June 24, 2024 – Vista Community Clinic (VCC), a leading advocate for community health and wellness, is proud to announce the launch of its new Diabetes Self-Management Education and Support (DSMES) program. This program is designed to provide individuals living with diabetes the essential tools and knowledge they need to manage their condition effectively and lead healthier lives.

Diabetes is a chronic condition that affects millions of people worldwide, and good management needs a thorough understanding of nutrition, physical activity, medication, and monitoring blood sugar levels. The DSMES program is hoping to address all of these needs through a structured and supportive approach, giving participants the skills and confidence to take control of their diabetes.

“We are thrilled to introduce the DSMES program to our community,” said Mackenzy Taylor, Certified Registered Diabetes Nurse Educator at VCC. “Our goal is to provide individuals with the knowledge and support they need to manage their diabetes effectively and improve their overall quality of life.” Cecilia Martinez, Registered Dietician at VCC, added, “Education is a critical component in diabetes management. With the DSMES program, we can help our patients make informed decisions about their health and live well with diabetes.”

Program Highlights:

1. Personalized Education: Participants will receive individualized education plans tailored to meet their specific needs, ensuring they understand the basics of diabetes management, including carbohydrate counting, insulin usage, and the importance of regular monitoring.
2. Expert Support: Program will be led by a certified diabetes educator, registered
dietitian, and healthcare professionals, who provide evidence-based guidance and support.

3. Flexible Options: The program offers both in-person and virtual sessions to accommodate different schedules and preferences.
4. Ongoing Support: After completing the initial education phase, participants will have access to ongoing support and follow-up sessions to ensure they stay on track with their diabetes management goals. The program is seven months long and offers a one-on- one appointment every month.
