Managing medical appointments, prescriptions, transportation and personal care can become overwhelming for older adults and their families. San Diego PACE provides coordinated health care and support to help qualifying seniors continue living safely in their homes.
PACE stands for Program of All-Inclusive Care for the Elderly. Operated by San Ysidro Health, the program coordinates a participant’s health coverage, medical providers and support services.
San Diego PACE offers more than traditional medical care. Services may include primary and specialty care, prescription medications, transportation to appointments, in-home assistance, rehabilitation, dental care, behavioral health services, nutritious meals and caregiver support.
PACE centers also include an adult day center where participants can enjoy activities, exercise, meals and companionship. A team of doctors, nurses, therapists, social workers, dietitians and other professionals develops a care plan for each participant.
To qualify, a person must:
Be at least 55 years old.
Live within one of the 114 ZIP codes served in San Diego County.
Meet the state’s nursing-facility level-of-care requirement.
Be able to live safely in the community with assistance from PACE.
People may qualify if they need help with medication management, diabetes care, bathing, mobility or other daily needs. Final eligibility is determined through an assessment.
For participants with Medi-Cal, or both Medicare and Medi-Cal, approved services are generally provided at no out-of-pocket cost. Those who do not qualify for Medi-Cal may pay a monthly premium.
San Diego PACE operates a North County center at 1840 West Drive in Vista. It is open Monday through Friday from 8 a.m. to 5 p.m.
