Vista, CA – According to the Centers for Disease Control and Prevention (CDC) in
2019 more than 1 in every 7 Americans is 65 or older. As our local community ages, Vista Community Clinic (VCC) continues its mission of Valuable Connected Care: by meeting the health and wellness needs of our seniors. VCC is thrilled to announce the opening of our Senior Focused clinic. This new site will be located at 105 Durian Street, Suite B in Vista, and will serve our patients over the age of 65 as well as VCC participants from San Diego PACE. PACE is an all-inclusive care for the elderly and has a brand new state of the art senior day center in Vista!
VCC’s goal is to have comprehensive health and wellness care accessible to seniors in their own community. VCC: Durian Senior Adult Care facility will offer a wide selection of senior focused services in addition to providing referrals to the San Diego PACE (Program of All-inclusive Care for the Elderly Program, if eligible).
Existing VCC patients over 65 are welcome to transition to the new site for their primary care and new patients in this age range will be offered to establish their care at this location.
